Diabetes Program Manager
Job Description
General Summary/Overview:
Responsible for providing professional nursing care for diabetic patients following established standards and practices as well as developing/teaching educational classes related to diabetes and weight management. Assists physicians and staff in coordinating patient care. Functions as primary liaison between patients, staff, and physicians
Principal Duties and Responsibilities:
- Plans a yearly schedule of classes and participates in marketing campaigns including making presentations, speaking on TV/radio shows and at other speaking opportunities.
- Develops weight management classes and related materials.
- Assesses the lifestyle and eating habits of individual patients. Determines appropriate diet/dietary restrictions.
- Teaches classes, individualizing as appropriate.
- Follows up with patients with phone calls, including problem-solving and positive reinforcement.
- Documents patient progress for medical records.
- Collects/analyzes statistics on program attendance, patient results and recommends changes.
- Provides general nursing care for patients. Administers prescribed medications and treatments in accordance with nursing standards.
- Observes, records and reports patient's condition and reaction to drugs and treatments of physicians. Dispenses medication as directed. Educates patient/family about diagnostic procedures, medications, nutrition and maintenance of health and wellness.
- Ensures appointment preferences are given to patients in emergency situations. Greets patients and prepares them for physician examination. Screens patients for appropriation information. Instructs patients about the collection of samples and tests.
- Responds to incoming patient treatment-related phone calls. Instructs patient and family regarding medications and treatment instructions.
- The job holder must demonstrate competencies applicable to the job position.
Qualifications:
Graduate of an accredited school of Nursing. Valid registration as an RN, BSN preferred, in the State of Massachusetts. One to two years experience in medical/surgical nursing and/or area of specialty preferred.
